Doctors make innovative vaginal reconstructions

Specialists in gynecology at the San Juan de Dios hospital built vaginas for two women who were born with congenital vaginal/uterine agenesis, conditions that prevent the uterus and vagina from developing.
 
According to Dr. Andrés Zamora, head of the gynecology department, vaginal agenesis is a rare disorder that occurs when the vagina and uterus do not develop. "Women with this condition have ovaries and normal external sex organs, but they do not have a vaginal canal or this can measure between two and three centimeters. These patients are usually detected in adolescence due to the absence of the first menstruation. Surgery can be performed to create the vaginal canal so that the patient can have normal sexual function."

The procedure has two parts: one surgical and one mechanical.  The hospital had to acquire a special device for each patient in order to perform this procedure.

This is the first time the surgery has been performed in North and Central America using laparoscopic surgery.
 
According to the specialists,  the two women had to use vaginal dilation techniques so that the tissue would be elastic enough for the surgeons to perform the procedures.

The incidence of vaginal agenesis is one in every 5,000 women, the most frequent cause being the Mayer-Rokintansky & Kuster-Hauser syndrome.

This condition is present before birth and may also be associated with renal, cardiac or skeletal abnormalities.

For Dr. Óscar Cerdas Salas, head of the Obstetrics and Gynecology section, "the most important thing about this surgery is to give these women the possibility of having a normal and healthy sexual life, with a very low surgical risk."

The procedure is known as Neovagina with the Vecchietti technique, an innovative laparoscopic surgery that provides a vagina through a rapid and minimally invasive surgical intervention.

The surgery lasts approximately two hours and is performed to place a surgical device that allows correction of vaginal agenesis.


According to the hospital, "two devices were purchased at an approximate cost of four thousand dollars each."

Women with this condition do not have a vaginal canal.


 
After the surgical procedure, the patients remain hospitalized for between 8 and 10 days. Each day, a traction is performed, which allows the vaginal canal to open a centimeter a day.

When the expected result is achieved, the device is removed. When patients leave the hospital they must use a vaginal dilator at night for a month after which the patient can have sexual relations.

Dr. Zamora, who is also part of the surgical team, said that compared to the old technique, this new surgery leaves no wounds or scars.

"There are about 11 surgical techniques described to create a vagina. Most of these techniques involve prolonged surgical time, extensive tissue dissection (abdominal and perineal incision, intestinal resection, large dissection in the perineum), with the increased risks due to the wide surgery." said Dr. Zamora.

According to the specialists, the results have been successful, the patients had no complications, no pain, a vaginal canal of 12 and 14 centimeters was obtained, and the external genital appearance was completely normal.

These surgeries were performed in the month of June, and after a period of medical follow-up, the surgery was announced as successful.


Government encourages use of electric vehicles

First lady Claudia Dobles announced, "a package of decrees and a directives" to encourage the use of electric transport, for both people and government institutions.

First lady Dobles confirmed that this package of decrees is the result of joint work among the Transportation Ministry, the Environment Ministry and the Finance Ministry. "We are encouraging zero emissions technologies, with actions aimed at meeting our de-carbonisation goals and allowing the population to join and make use of an increasingly sustainable and efficient transport."
 
The new decrees announced by the government are:

- Incentives for Used Electric Vehicles.

- Modification of the Regulation of the Law of Incentives and Promotion for Electric Transport.

- Promotion of Sustainable Mobility in the Central Public Administration.

- Repeal of the Decree that Encourages the Use of Hybrid Vehicles.

- Guideline for the Transition to an Electric Vehicle Fleet or Zero Emissions in the Public Sector.
 
The Incentive for Used Electric Vehicles is “an economic prize” for the purchase of used electric vehicles no more than 5 years old. The objective is to encourage the use of this zero emission technology in the middle income population. It forgives the Selective Consumption Tax, which corresponds to 30 percent of the value in customs. This applies to vehicles with a value of no more than $30,000, stated the government.
 
Additionally, these cars will not be subject to vehicular restriction and may park in spaces designated as blue within public parking lots and in private parking spaces located in supermarkets, shopping centers, etc.
 
The Guideline Transition to an Electric Vehicle Fleet in the Government was created for government institutions that purchase only electric vehicles.

The Decree Modification to the Regulation of the Law of Incentives and Promotion for the Electric Transport allows a registry by model of automobiles and verifies of the qualities of the vehicle would be approved in a shorter term.
 
The Repeal of the Decree that Encourages the Use of Hybrid Vehicles allows tax exemptions to be maintained for 12 months, to allow importing agencies to comply with their annual projection. It also seeks to discourage hybrid cars because the government now believes it should move directly to the replacement of internal combustion engines.
 
The Decree Promoting Sustainable Mobility in the Central Public Administration establishes that each institution should create a general support plan for employees who choose not to use the car or bus to go to work. As for example: conditioning showers for those who travel by bicycle, installation of parking lots for bicycles, encourage teleworking, etc.

According to the government's plan, "these measures will improve the quality of life and efficiency of public officials, reduce emissions and traffic congestion, encourage the use of public transport and save fuel."

Turrialba Volcano ash reached Central Valley

The National Commission of Emergencies has confirmed ash fall from the Turrialba Volcano, reached the Central Valley.

According to the report of the Volcanological Observatory, the emanations of ash and gases started at 7:45 a.m. Tuesday. "This is one of the longest ash emanations in the last 8 days, the volcano continues with explosions and ash emissions, and you can see the incandescence material around the crater."

Experts stated that the direction of the wind has been maintained towards the west – northwest and that the decrease in rain and the increase in winds favors the ash reaching more populated areas of the Central Valley. The fall of ash has been reported in the vicinity of Cartago, San Jose, Heredia and Alajuela.

The commission is calling on the population living near the volcano to be vigilant due to the continuing gas and ash emissions.

The Emergency Commission also stated that anyone who has a respiratory health condition with chronic problems should take the necessary measures to avoid respiratory problems.

Favorable weather conditions have allowed volcanologists to observe the volcanic activity through the monitoring cameras located in the area of the viewpoint and in nearby villages.

According to specialists, since the end of August 2018, "the Turrialba Volcano has increased its activity and has been emitting ashes and gases more persistently."

It is very possible that there will be more eruptions similar to, or even stronger than, those that occurred in the last hours, said specialists.

Turrialba Volcano is an active volcano in central Costa Rica that has been explosively eruptive since 2016. It is located in the vicinity of the city of Turrialba in Cartago and has an elevation of 3,340 meters. According to the observatory record, it is about 1.5 million years old. The surrounding volcano area is the Turrialba Volcano National Park.

‘Trust for the Americas’ will donate
$105 thousand to rural area projects

The non-profit organization ‘Trust for the Americas’ announced their call for funding projects in rural areas. Organizations that have projects designed to help the community may apply. 

According to the organization, the program is known as, "From Open Government to an Open State in Costa Rica," and is aimed at financing projects of community organizations, media, representatives of the education sector or the business sector.

The objective is to present social aid projects in partnership with a municipal entity.

The organization will donate up to $7,000 to 15 selected projects. The objective is to finance cooperation projects with the municipalities and the community of the regions in San Ramón, Esparza, San Mateo, Orotina, Quepos, Bagaces, Cañas, Pococí, Jiménez, Hojancha, Abangares, Turrialba, Guácimo, Osa and Grecia.


‘Trust for the Americas’ is affiliated with the Organization of American States (OAS) and has more than 15 years of experience in creating initiatives that seek to improve access to economic opportunities and contribute to citizen security for the vulnerable populations of the hemisphere.

Education Ministry will delay
payment of extra salaries to employees

The Education Ministry confirmed that, due to the delay in the delivery of the reports of overtime or surcharge of functions of employees, there will be delays in the payments of those amounts.

According to Ministry, the system of surcharge of functions and overtime is a way to attend to fundamental tasks in schools. "On average, around 30,000 of these employee bonuses are recognized each year," said Ministry statements.

The report of these extra wages had a deadline of November 2. The directors of each institution are responsible for the delivery of these reports. Apparently, only 3,000 reports were received, equivalent to 18 percent of the estimated total of 16,000 reports expected.

For Yaxinia Diaz, director of Human Resources, "the biggest problem that is generated from this situation is that, sooner or later, the avalanche of late requests will arrive."

According to Diaz, the Ministry has many other tasks to fulfill, in addition to reviewing those reports, such as appointments of new professors, requests for transfers of professors, and employee disability procedures, among others. For those reasons, the payments will not be ready when employees expect.

According to the Ministry, thanks to the extra payment employees could receive up to an additional 50 percent of salary.

Apparently, the principals of the schools did not deliver the extra payment reports on time, because they were on strike.

The strike of teachers and other ministry employees was declared illegal by the Labor Court of San Jose. However, "the education sector represents 98.55 percent of the people who continue on strike since September 10," confirmed the ministry.

Police arrested six tourists for not
declaring more than $57 thousand

Police of the Juan Santamaria Airport detained six tourists who tried to enter the country with $57,610 hidden in their luggage.

According to the police report, the six Mexican tourists detained were identified with the surnames: Valderrama, age 34 carrying $9,760, Torres age 42 carrying $9,440, Álvarez, age 54 carrying, $9,704, Acosta age 36 carrying $9,826, Garcia, age 50 carrying $9,400, and Estrada, age 24 carrying $9,480. 

Migration protocols state that, "everyone must declare sums greater than $10,000 in cash," but when groups of people travel together, "the money is shared to evade controls at the airport."

Police state that this sharing of money is often a way to hide the fact that the cash could be related to money laundering proceeds from drug trafficking.

The money was detected by a canine agent names Sonya who alerted police after sniffing the luggage. Analysis of the bills showed that they were positive for traces of drugs.

The six detainees were sent to prosecutors on suspicion of concealment of capital allegedly obtained from drug trafficking.

Travelers must declare any amount in cash to customs.


There they will be interviewed as part of the investigation process and then they must wait for a judge to decide their legal situation.

The money seized will be delivered to the Costa Rican Institute on Drugs, the state agency in charge of managing the seized assets.

My recommendation to live longer, healthier & happier

Dear A.M. Costa Rica readers:

On the story published yesterday from the United Nations points out that hunger, malnutrition, lack of micronutrients, overweight and obesity affect low-income people, women, indigenous people, Afro-descendants and families more often in the rural areas of Latin America and the Caribbean.

The solution is simpler than you think. People need more education about healthy food.

Healthy food can make more positive changes in people's lives in a fraction of the cost of those campaigns from multi-millionaires organizations like FAO.

This is my motivation to share with you the story of Melissa Guzman.

Born and raised in Costa Rica, Melissa Guzman has always had a passion for great and healthy food. As specialist on healthy food Melissa would often have requests from people asking her about some of her recipes. She has published a book, with the best and most delicious healthy recipes. “Living Longer, Healthier & Happier” is a collection of traditional and modern Costa Rican recipes made and recommended by Melissa. The book features large and professional photos of each recipe. But beyond the recipes, the narrative focuses on much more. The book and its recipes is really about the Costa Rican lifestyle, what Ticos refer to is the “Pura Vida,” which focuses on a healthier and happier lifestyle all of which is centered around the food and culture of Costa Rica. While the recipes are one step to a better well-being, the book features a number of tips to living longer, healthier and happier as she said.

For example the gallo pinto recipe, that is as traditional as it comes in Costa Rica, but have an option to use quinoa instead of rice.  Tastes amazing and it’s even healthier.

A well-balanced diet provides all of the energy you need to keep active throughout the day and nutrients you need for growth and repair, helping you to stay strong and healthy and help to prevent diet-related illness.

Keeping active and eating a healthy balanced diet can also help you to maintain a healthy weight. Deficiencies in some key nutrients - such as vitamin A, B, C and E, and zinc, iron and selenium - can weaken parts of your immune system.

Maintaining a healthy weight and eating a balanced diet that's low in saturated fat and high in fiber found in whole grains can help to reduce your risk of developing type 2 diabetes.

A healthy diet rich in fruits, vegetables, whole grains and low-fat dairy can help to reduce your risk of heart disease by maintaining blood pressure and cholesterol levels. High blood pressure and cholesterol can be a symptom of too much salt and saturated fats in your diet.

Eating a portion of oily fish - such as salmon and trout - each week can also help to lower your risk of developing heart disease. The high levels of omega-3 fatty acids in oily fish are good for heart health.

Eating a healthy diet and a moderate amount of unsaturated fats, meat and dairy can help you maintain a steady weight. Having a good variety of these foods every day leaves less room for foods that are high in fat and sugar - a leading cause of weight gain.

Melissa's book is about embracing the principals of the Blue Zone and a big part of that is a healthy diet, but we also stress the importance of a healthier and slower lifestyle that is all about living the Pura Vida we know so well in Costa Rica.

Authorities identified a woman who was killed and mutilated

The Judicial Investigation Agency has confirmed that the mutilated woman found in Bebedero de Escazú was 28-year-old  Stephanie Castro who lived in Desamparados.

The police report states that they were alerted by a man who reported that he found his dog dragging a human arm. When the team of detectives and the police arrived they found other parts of the body spread out in an empty lot. The skull was found next to a power line post very close to the road.

Forensic specialists suggest that the woman was murdered elsewhere and her body parts dumped in a desolate place. Analysis indicates that the body was dismembered with a tool of some kind and that the murder could have taken place three or four days ago.

According to the statements of Walter Espinoza, director of the Judicial Investigation Agency, "the woman had no criminal record and was identified by means of some tattoos."

According to the police report, the forensic experts are still searching for body parts.

The investigation of the unusual crime is in the initial stage of analysis.  Espinoza stated that a team of specialists are researching the case and no further details can be given out at this time.
